Referring MDs

We are currently accepting referrals for both Obstetrical and Gynaecologic care.  We would appreciate receiving the referrals by fax, along with all of the necessary supporting documentation.  For your convenience, separate referral forms for Obstetrics and Gynaecology are attached in PDF form in the sidebar on the left.  We have also attached a one-page patient information PDF for patients who have been referred to our clinic.  Feel free to download and copy these.

Please remember to attach all relevant supporting documentation (ultrasound reports, bloodwork, cultures, Pap reports, antenatal forms, etc.) as receiving this information before the patient’s appointment is much appreciated.  If information is missing, valuable appointment time is wasted calling for reports.

You may refer to a specific physician.  If you check more than one box, the patient will be booked with the earliest available physican amongst those selected.  You can also check the box entitled "CLINIC TO DESIGNATE", in which case your patient will be triaged to the most appropriate physician, according to the patient’s diagnosis and the urgency of the situation.  

Some of the individual physicians in our clinic also triage their own referrals according to diagnosis and urgency.  In order for your consultation request to be triaged appropriately, it is important that you provide as much information as possible.  If your referral is URGENT, please indicate this on the referral form.  In most cases, it will be possible for your patient to be seen within 3-4 days.  If your referral is MORE URGENT than this, please call to speak to one of our OB/GYNs, as your patient may need to be directed to one of our hospitals for urgent care. 

Once your patient has been assigned an appointment date and time, your office will be notified by fax. 

Please note that we will not be contacting the patient directly and that it is the responsibility of your office to ensure that your patient is aware of her appointment details.  The patient must also confirm her appointment one week prior.  Failure to do so may result in appointment cancellation.  Confirmation is best done by e-mail, but can also be done by phone.  We require 48-hours notice for appointment cancellations and changes and we reserve the right to charge the patient a fee for missed appointments.
